To Schedule Your Assessment via the mail, please carefully follow these instructions with your order:
- Include with your order your full name & mailing address.
Your card will be charged at this time and we will send you out the supplies and instructions needed, along with some suggested dates for taking your samples.
- When you receive your package in the mail , please read all instructions on how and when to collect your urine and saliva samples.
- Contact us to schedule the date that you will send your samples. (Time is of the essence--do NOT just take your samples and send them--this MUST be coordinated with our office!).
- If you do NOT have e-mail, once we run your samples, we will call you to schedule a phone appointment to go over your results and the rest of your information will be mailed to you. If you DO have e-mail, Frankie will contact you via e-mail with her personal input and send you your report and other information electronically.*
*If you have e-mail but choose to have a phone appointment with Frankie as well, this can be scheduled at no charge (you will be expected to make the phone call to Frankie direct).
